Physical therapy programs pose great demands on patients, especially those who have never exercised before but who are determined to work hard to rebuild their bodies and their lives. Patients rarely have trouble adhering to their rehabilitation program while in their regular therapy appointments, however, after completion of their PT program, it can be difficult for patients to continue their prescribed exercises without the equipment, structure and trained personnel that was previously available to them. Once discharged from physical therapy, patients are left to rely on themselves to adhere to post-rehab exercise programs outside of the clinic, and some neglect to follow through. |

Unfortunately, how well and how quickly patients completely recover from their injuries often depends on how well they follow their programs at home after being discharged from therapy. In addition, patients may also want to consult with therapists with any questions or concerns they while away from therapy.
FVPT has surveyed our patients and developed our Transitional Therapy Program in response to this need. During or after their time of discharge from physical therapy, patients meet with a therapist to develop an exercise plan, including descriptions, illustrations and demonstrations of each activity. The patient can then return to FVPT, have access to the exercise equipment, and consult with our trained staff for up to four months after discharge from PT, at a reasonable cost. Using this program, patients can gradually shift to an independent, long-term exercise program for optimal health and wellness.
